time registration:
- 2 photos (with
last name, first name and club on the back of the
photo).
- Sending Form
of the licenses (with seal of the doctor + seal of
the president of the club).
- Birth certificate.
- Contribution
(herewith sending form of licenses prices category).
For a renewal:
- Photocopy of
the old license 2006-2007.
- A photo (with
name, first name and club to back) for the players
that pass from a category to another.
- Sending Form
of the licenses (with seal of the doctor + seal of
the president of the club).
- Contribution.
Important
Remarks:
- The deadline
for the regulation of the contribution of the affiliation
is set up for 30 November 2008
- THE ACCEPTANCE
and the deposit of the licenses is set up according
to stopped periods as follows:
- 1st period:
of November 03 to November 30 2008
- 2nd period:
of the 05 to January 31 2009
- 3rd period:
of the 16 to February 28 2009
- 4th period:
of the 13 to April 30 2009
- 5th period:
of the 18 to May 31 2009
- 6th period:
of the 15 to June 30 2009
